This is for a manual enrolment for an INVOICE—this can be passed to your accounts team to process for an Purchase Order or paid upon receipt. We will email the invoice to the contact person advised on the following form. Upon full payment of the invoice we will set up the trainees to the courses advised on the form.
When completing the trainee section on the form please provide:
State which course they are enrolling for ie. Safe Use of Lasers OR Safe Use of Lasers & LSO course.
Advise the trainee name(s) how it will appear on their certificate, and the best email address for each trainee. The email provided will be used to log into our Litmos Learning Portal. PLEASE NOTE: a work email might have restrictions so a personal email is preferred.
